This assessment is commonly used to support NDIS planning, plan reviews, and funding decisions, giving a clear, evidence based picture of a participant’s functional ability and core support needs.
This includes looking at equipment, home modifications, and other supports that may be required, so the resulting report gives a complete picture of funding needs, not just a summary of diagnosis.
The assessment considers a person’s home, community, and social environment, along with their personal goals, to build an accurate picture of the support required for safe, active, and independent living. Available by telehealth Australia wide or in person across the Perth metro area.
