Functional OT Australia prepares occupational therapy assessments and reports that support a range of NDIS processes, including Functional Capacity Assessments for planning and funding decisions, Home and Living Assessments for Supported Independent Living and Specialist Disability Accommodation, and Review Reports for plan reassessment.
Our reports are written to give clear, evidence based information about a participant’s functional ability, support needs, and goals, helping to inform NDIS planning and review processes.
We work with participants, families, and support coordinators throughout Australia, with telehealth available nationwide and in person assessments across the Perth metro area.
